Investigation Meeting

I have been invited to a investigation meeting due to me leaving early one shift to a doctors appointment and forgetting to clock out in the process. I am aware this was stupid and I should have informed my manager, but he/she wasn't present that day and I have been stressed out recently because of an elderly family member being ill (covid) so it slipped my mind to inform them that evening . I know I am in the wrong, and i hold my hands up. But I am wondering what the likely outcome of this meeting will be, am I in serious risk of loosing my job?
